12 tradelines on CRA's from same collection agency, ?'s about letters

I am helping my brother with his credit repair.

 

He has 3 medical collection accounts all with the same CA. These report to all 3 with the exception of Experian with is reporting a duplicate account for each one for a total of 6 tradelines. These duplicate accounts were previously removed from Equifax and now appear on Experian. These accounts are SOL with a DOLA 8/2008. If I have done my research correctly I think the best letter to use is the "SOL PAST 4 YEARS".

 

Questions - I would need to send a total of 12 letters to the CA? When would I send the "FOLLOW UP CRA" letter? Only if I don't get a response?

Once he has opted out and deleted old addresses then the initial dispute letter that goes to each CRA should list ALL medical accounts exactly as they appear on the report being disputed.

I have done the above and nothing 3 get deleted from one and added to another. It's been about 6 months since the last time, should I do it again?
