Yes, file a complaint against the CA with the FTC and the CFPB

Send a copy to the CA and send a copy of that complaint along with the copy of each CRA complaint to Eq and Ex

 

In THAT complaint, stress that the last time you were treated at that facility was in 2004 and that the CA refused to provide you with any kind of verification that the account was yours.

Just to clarify, should I ONLY say that I had treatment back in 2004 in the CA complaint or in all 3?

You can say that you had treatment at the facility in 2004 that was totally covered by your insurance with no balance owed.
