My husband, living in England, was always taxed as single and childless and had the highest liability without offsets.

 

He paid about £40 a month flat, no fees for anything else. Every £9 max, usually less.

 

When I was there I paid out of pocket for a visit and prescription, and out of pocket was less than my co-pay in the US, my rx was also cheaper than generic with insurance.
