It says to include a payment for original balance. https://whychat.me/hipltr.html That is for accounts that are relatively recent where the date of service is less than 2 years old (that is the usual cutoff date for HC providers to migrate records from their active files to their archives) AND when the HIPAA letter program has documented a CURRENT BUSINESS RELATIONSHIP between the reporting CA and the OC.
