Need help with student loan

A very long story short. Orginal loan was 40K taken in 1999.  I have paid a total of nearly 55K over the years and my balance is now (down to) 97K. I'm bipolar and have lost my job a bunch of times and used all forbearance etc.  Capitalization has run my balance way up. I consolidated my loan 15 years ago bringing interest rate from 10% to 7.5%. My loan has now defaulted and I attempted to initaiate a rehab because I now have a job. They have taken 3 payments but are arguing with me saying they didn't receive my papers in time. The guarantor disagrees. Credit coleection costs of 10K have been added to and are part of the 97K. I have no idea what to do but think the situation is absurd. Any advice would be much appreciated on how to resolve this most beneficially.

If this is a federal loan, can you apply for disability discharge on this loan?
