No, the ONLY time you need to send proof of your identity is at the beginning of the program when you are disputing addresses.

EQ is being stubborn.

I have received no response by mail but, again, an email stating that my dispute has been resolved.

I checked the backdoor and it states "consumer disputes after resolution" but no deletion.

I have not received a response from the CA.

What are my options to get this last one deleted?

Try following the guides:

http://www.whychat.5u.com/GUIDE%20HIPAA%20PROGRAM.html

The next step would be the medical DV to the CA(s)

http://whychat.5u.com/ltrcavalhipaa.html

and the follow up dispute to the CRA(s)

http://whychat.5u.com/ltrcavalhipaa.html#DISPUTE

Please post ONLY your results or urgent questions, we don't need a blow by blow account of your activities

Dear WhyChat.

I have followed all those steps in order to a tee. I was successful with EX but not EQ. Please see up thread.

If having followed all those steps and still unsuccessful, are there any further actions I can take?

OK, you opted out, deleted old addresses, sent the initial dispute to the CRAs sent the medical DV to the remaining CA, sent the follow up dispute to Eq who is the ONLY CRA still reporting.

The next step is to file a complaint against Eq with the CFPB

http://whychat.5u.com/hipaaftccomp.html

Be sure to state in your complaint that you properly disputed an unknown medical account and followed up with additional requested communications to the reporting CA and Eq but that Eq has refused to properly investigate and delete. That Eq is the ONLY CRA reporting this unknown medical account.
