I have a Chex Report form 04/08. Since then I have paid a Chex item. I was going to try to dispute the item off, but should I obtain a new report before disputing? Do I have to be denied an account to obtain one? I can't remember if I got this one b/c of a denial or b/c of the free annual one.

no, you do not need to get a new report to file a dispute.... there really is no need

Ok. I got a new report anyway. I have 2 items. One is from my current bank and is a paid item. I guess there is no way of reporting as a paid item on Chex, but it will just remain for 5 years. The other is an account that I remember having with a bank, but don't remember there being any NSF charges after the account was closed.
Can someone point me in the right direction on how to dispute these items? I know nothing about Chex or the proper way to dispute.

Is your Chex reflecting a paid status for the amount owed? If not, then they have to correctly update that status or delete!
