I had an ER charge of around $3200 that was severely inflated as being uninsured. I obtained my medical records and forwarded the billing and medical records to a patient billing advocate. The advocate reduced the billing to around $1300 as being reasonable and customary. The account is in collections.

With the HIPPA program, how would I offer to pay this reduced amount?

The HIPAA letter program, insert "a" has the appropriate language to use, here is the paragraph modified for your purpose.

(insert a)
Enclosed please find my remittance of ($___) for payment in full of this account.
This payment in full is for services as per the attached fee schedule from (XXXX XXXX)Include the documentation from the patient billing advocate)

Please note, my remittance is payable ONLY to (hc provider) and may not be signed over or transferred to any third party collection agency, as this would constitute an additional violation of HIPAA and State Privacy Act rules .
Copies of this correspondence and a copy of the remittance check may be used for any further actions with State or Federal agencies
