I don't have one. Is there a good form somewhere?

in excel I simply add all assets and debts and then subtract the latter from the former.

you can avoid excel by using the free spreadsheet from openoffice.

For my Personal Finance class we had to do a project involving one. THis is what it ended up looking like (she gave us the template):
