SO has a paid collection account to UCB Collections for $42, Date of status 10/03.
It went to collections, but she paid the OC directly in 2003 because they would not allow her to have a second surgery until the balance was taken care of. She is positive that she did not deal with the CA about this at all.

I know the first step will need to be the Pre-HIPPA medical dispute letter to the CRA. Drafting that right now, but if it doesn't get deleted there, is this an instance in which the HIPPA dispute process would be applicable? I know that you're supposed to pay the OC directly when you go the HIPPA route, and she did, just 3 years ago...

Yes , if it was paid to the OC, then the HIPAA letter insert "b" would apply. Do the medical dispute letter first though. Make sure you have opted out and correct the addresses.
