I did it all in the same letter. It falls under the "personal information" category with the CRA, so here's how I worded my letters:

2. My current address is:

Please remove all other addresses IMMEDIATELY. They are not accurate.
3. My address does not pertain to any business. Please update your records to reflect my address as a personal residence.
4. My employment information is: XXXXXXXX . Please remove all other variations IMMEDIATELY. They are not accurate.
5. Please delete all phone numbers IMMEDIATELY. They are not accurate.
Please delete these disputed items within thirty (30) days, which constitutes a reasonable amount of time for your investigation. I have attached the necessary documents to verify the above information.
If you are unable to remove the erroneous information in any way, please provide the name and contact information of the data furnisher for each incorrect entry.
Per the Fair Credit Reporting Act, send me notification that these items have been deleted, as well as an updated copy of my credit report once your investigation is complete.

Thanks so much for the info but I do have a question and pardon my ignorance.....Is there a reason why you would not want to include your signature on correspondence to a CRA and is it a best practice to not include it?
