The only requirement is the biller have one of the 5 MCC codes (previously listed).

I didn't read through all the recent posts, but I didn't see anyone confirm the date for the cap of the bonus $25 on $100 redeemed to once a year.
I was on the phone with a CSR just now and she seemed to be reading from an info sheet that stated that the cap of the bonus goes into effect July 1. She also had the impression that any bonuses earned and redeemed before July 1 will NOT count for your yearly bonus. Get your bonuses now guys.

Someone posted in a thread of mine that this card was getting devalued in Q2 this year? Can someone explain what's happening to the card? I'm thinking about applying for it this week.

That sounds accurate because I called as well and they told me I would get $25 every time I redeem $100. I said I heard you wernt doing that anymore and she said no we still are.. So I don't know what that means or how long it will last..

Can Credit Card Rewards Be Too Generous?
By now, it is easy to see where this offer would be going. According
to Teri Charest of U.S. Bank Corporate Public Relations, starting in
July, US Bank will be making significant changes to this program as the
5% cash back categories will change. Charest explained, “We are swapping
out the home improvement, airlines and bill pay with the new categories
book stores, sporting goods and cell phone purchases.” In addition she
explained that “the $25 Cash+ bonus will be limited to one starting in
July.”
So until July comes, enjoy the card as is.
When July comes, it'll still be awesome to fill the gaps in your categories for other cards.
The loss of the 25% bonus is disappointing.

Yeah, combined with the fact they swapped out the 2 best categories IMO (HI and BP) makes this card just ok. On the plus side, it allows me to always have a 5% cash back on restaurants card, and 5% off my cell phone bill. What will be interesting to see is if the amazon purchases get coded as bookstore like they do on the Sallie Mae card...if that is the case, then this card becomes very good again for me.

I'm looking at the gold/platinum checking packages on US Bank's site about the rewards bonus:
Available until August 10, 2013, earn a 25% monthly bonus on rewards earned for net purchases (purchases minus credits and returns) with a U.S. Bank Cash+™ Visa Signature® or FlexPerks® Visa® Credit Card.
Available until August 10, 2013, earn a 50% monthly bonus on rewards earned for net purchases (purchases minus credits and returns) with a U.S. Bank Cash+™ Visa Signature® or FlexPerks® Visa® Credit Card.
Does this mean the bonus only applies until August 10? Deciding whether or not it's worth opening a new account there now for the bonus.

Are restaurants and fast food really categorized differently?
I looked up McDonalds on the VISA MCC lookup and it's categorized as a Restaurant. Can I get away with just picking Restaurant for my bonus categories, or should I get Fast Food too? Can anyone confirm if something like McDonalds would qualify for Restaurant?

They are definitely categorized differently, and I did NOT get 5% when I used my card at McD's (along with a lot of other fast food/deli type places)
