They had a tough time finding it when I called about it (I played dumb). They've since changed the name of the bank and I have changed my last name since then. Should I send a copy of my SS card and DL with my new name and dispute as "not mine" to try to get it deleted?

You need to dispute with the CRAs first, in order to do a 623 with the original creditor. The good advice I've followed in the past is to exhaust all avenues of dispute with the CRA before contacting the OC.
